What is a reasonable accommodation form?

A reasonable accommodation form is a a verification document that sometimes a landlord will require to be completed by a therapist who has evaluated you. This form is not to be confused with our ESA letter.  

Is there a fee for this service?

ESA Registration of America charges a $75 fee for this service. 

How do I submit my RAF?

Please note only those who have already gone through an evaluation with ESA Registration of America will be able to use this service
