How do I schedule my Appointment?

Your evaluation calendar will appear after you submit your assessment form however if you decide to come back to it at a later time you can use this link to access your current evaluation and go from there.

Once you open your assessment you can click on the schedule now button if you already finished answering your questionnaire.

1. Select your time zone.

2. Select an available date + time. 

3. Provide your cell phone number to get a text reminder.
